Anthem agrees to $12.8 million settlement — here’s who can claim a payment

Anthem, now Elevance Health, has agreed to a $12.875 million settlement in a class action lawsuit. The suit alleged the insurer denied coverage for residential treatment services by using overly restrictive criteria. Eligible customers whose claims were denied between April 29, 2017, and April 30, 2025, can file for a pro rata share or a nominal payment.
